The recurring-problem checklist
Identify one repeated operating problem and the evidence needed to address it.

What you’ll work through
A problem that happens again deserves a closer look. Choose one recurring issue from the last working week. Use this checklist before writing another procedure or buying another tool.
- Check the evidence
- Check the arrangement
- Choose one action
- Review the original problem after the trial. Record whether it happened again and what changed. If it continued, revisit the cause before adding more instructions.
A look inside
Check the evidence 1. Describe the last occurrence: what happened, when and who owned the work? 2. Find the record or recent example. Mark a missing fact as unknown. 3. Count separate occurrences during a defined period. Do not count three messages about the same incident as three failures. 4. Record the time spent resolving each occurrence.
